En este video se puede analizar claramente que eran requeridos varios puntos:
– Tener un supervisor experto de reemplazo que apoyara al operador de refineria.
– Calibrar apropiadamente el sensor de nivel de la torre.
– Verificar que la alarma de HIGH HIGH se activara apropiadamente. No conocemos si en esta torre se habían hecho trabajos de Alarm Management (aplicación de la ISA 18.2), cuyas mejores practicas indican que la alarma de HIGH HIGH se elimine, y se deje tan solo la alarma de HIGH, con PRIORIDAD ALTA.
– Tener indicacion en la misma HMI de los flujos de entrada y salida de la torre, para que el operador pueda establecer con simples sumas y restas si se está acumulando material en la torre ó no.
– Un sistema de Deteccion y Diagnostico de Fallos pudo haber ayudado para analizar la alta presión en la cima que se presentó a las 12:41 pm, y recomendar otras acciones diferentes a abrir el venteo de la torre.
– Detectar el Fallo de la alarma de Alto Nivel del tanque de purga.
– La valvula de control automatico de nivel de la torre (salida) se dejo bloqueada durante el arranque, y cuando se habilitó, calentó el líquido de entrada a la torre que ya estaba inundada.

The explosion and fire of the BP Refinery in Texas City, the tragedy with the largest number of fatalities in non-residential properties, occurred in 2005. The counter-explosion of the engine of a truck ignited a flammable hydrocarbon liquid and steam from an atmospheric ventilation of the isomerization unit (ISOM).
In this video it can be clearly analyzed that several points were required:
– Have an expert replacement supervisor to support the refinery operator.
– Properly calibration of the tower level sensor.
– Verify that the HIGH HIGH alarm will activate properly. We do not know if in this tower they had done Alarm Management works (application of ISA 18.2), whose best practices indicate that the HIGH HIGH alarm is eliminated, and only the HIGH alarm is left, with HIGH PRIORITY.
– Have an indication in the HMI of the input and output flows of the tower, so that the operator can establish with simple addition and subtraction if material is accumulating in the tower or not.
– A system of Fault Detection and Diagnosis could have helped to analyze the high pressure at the top that was presented at 12:41 pm, and recommend other actions to open the vent of the tower.
– Detect the Failure of the high level alarm of the purge tank.
– The automatic level control valve of the tower (output) was blocked during start-up, and when it was enabled, it heated the inlet fluid to the tower that was already flooded.
